Rod One: This place is absolutely DISGUSTING! I Highly advise you NOT to allow any social worker from any hospital to convince you into referring that place. The nurses are completely unprofessional, and do not care whatsoever about what happens to your loved ones. I personally had to clean manure from my Mothers restroom. I also had to clean urine from the floor. It is ran by people who have deep language barriers. I’ve been to better holes in the wall than that place. They steal and they take their frustrations out on your loved ones. I reported them to the Board of Health. Let me not forget that it looks like an insane asylum than a short term rehabilitation facility. I also believe it does not deserve a one star rating.

Jennifer Gunderman: My father received stellar care at Hermitage House. The staff were caring, compassionate, and hard working. The OT and PT were patient and encouraging. The nurses and nursing aids were kind despite my dad's less than sunny disposition. The social worker was timely and honest in her communication. The housekeepers worked diligently to make the facility clean, safe, and welcoming. The one challenge is the age of the building but staff cannot change that.

Aida Clemmey: I gave it one star just because the 3 residents at the nursing home that told my family that told us that the staff at Heritage would stick my father in a wheelchair and sit in at a table from when he gets up in the am until 9:30pm when they it him to bed... The male staff that took care of my father would all way complain when I or my family would take him to go bathroom... He would even walk away when he would see me bringing him down to his room to go to the bathroom... And wouldn't even let him take naps during the day... They even had information that he had kidney failure 4 day until we saw the report. And went to the hospital.... So it's all up to you if you want to send your love one to a place like this you must not love them... The staff is lazy...

Phylicia Burgos: My mother in law has been a resident here and recently she asked to have her hair done by the hair dresser and the hair dresser said she doesn’t do black people hair..this isn’t ok and something needs to be done about it
NIGEL BELGRAVE: The ER was very understaffed resulting in long wait times. Nurses were clearly overworked. Rang call bell on multiple occasions and did not recurve assistance for over 45 minutes! Not enough rooms in ER resulting in patients waiting in hallways over night. Inadequate spacing of patients would do little to prevent the spread of airborne illnesses. Insufficient parking on grounds resulting in having to park on lawn and unmarked spaces. obvious disregard for family members which negatively impacted care of elderly patient. Would recommend seeking help elsewhere whenever possible !

Laurie Smits: My elderly mother spent 50 days at the Grafton area Saint Francis Rehab Facility where she received excellent care from their nurses, CNA's, physical therapists and occupational therapists. I visited daily and witnessed her quality of care. She received physical therapy 5 days each week and occupational therapy 7 days each week. The amount of time in therapy was important to us and we were very happy that she was actively involved daily. All therapists worked closely with her targeting specific needs. They provided extra exercises for her to practice on her own, whether she was in bed or in the wheelchair. She enjoyed going down to the gym for higher level therapy. Most importantly, they applauded her successes and communicated her strengths and weaknesses with me. The entire staff at Saint Francis were very friendly and professional. They all worked tirelessly for the patients and asked if they needed anything else. There were times my mother needed help with everything: eating, sitting up, adjusting the bed, dressing, using the restroom, and more. She was frustrated she couldn't help herself more. This staff always helped her in a friendly manner and always let her know it was ok, not to worry. Even the girl cleaning her room would address her by her first name, ask how she was doing each day and talk with her for just a couple minutes. The Caseworker would come by and check on my mother and discuss her progress and options. Even if I walked by the Priest outside, he would ask how my mother was doing. The Saint Francis community took a genuine interest in my mother and truly cared about her. These few minutes of conversation made my mother feel like she was part of their family and gave me peace of mind. For food, they have a menu selection for lunches and dinners. If you don't care for the menu items, they always have some food available that you can request anytime: soup, grilled cheese, chicken tenders, sandwiches, hamburgers, cheeseburgers and more. While this facility may not be as new as some others, it is clean and well maintained. What truly makes it special are their staff members. Thank you Saint Francis for all you do!
Jeff Bailey: Awful communication, awful care and things have become worse since he went in. He may have even had a psychotic break. Right now, I do not know if he is alive, dead or some other facility.

When she passed on June 22, 2022 (her name was Marie Rock) they called me everyday to make sure I was ok I think they went above and beyond. I would recommend them to everybody that is looking for a good nursing home in Worcester Ma.

Cai Mickey: If you care at all about your family members you will not send them to this place. My father in law was here and has missing clothes and shoes, wasn’t showered or had his face shaven. When sent home they didn’t adequately transport him into his vehicle. They sent us home with another patient’s medical information (HIPAA violation). They set him up for a medical procedure at the hospital without telling us which we were fortunately able to cancel. Awful place that needs to be investigated by the state.EDIT 12/3/21: I just saw the response from this place, don’t worry. You’ll get to talk to me about this when the state board investigates you. Your inadequate care played a huge part in my father in law’s failing health. I find it funny that my negative review is the only one with a response- because you know you messed up BADLY. Rebecca Sarja: My grandmother is currently here and hasn’t been there long but the experience is terrible. It’s very gross inside. She’s already caught the nurses trying to get into her computer she has there. They only shower her once a week, every Wednesday but for the past two Wednesday’s they’ve told her she can’t get a shower because they have no hot water. TWO WEEKS without a shower. Most of the nurses have a nasty attitude. I will forever wish i could bring her somewhere else or to live with me because this place is horrid.

Colleen R: My mom was there several years ago and the care was awesome. However recently mom was in Notra Dame Rehab after being discharged from the hospital and had a lot if problems going to doctor appointments. They constantly cancelled them due to transportation. Her shoulder got injured while she was there also. Without going into detail I will be sure she will not be sent there again.I do want to add that the PT and OT women are awesome and are not part of the problem.
Karen Hogan: I had the pleasure of visiting Notre Dame recently and was so impressed. A beautiful, peaceful campus and friendly, happy staff and residents. The recent renovation to their Assisted Living spaces are fantastic. Can’t say enough good things about this community.

Joseph Bagnell: Notre Dame has a very caring staff. There priority is the resident. I know nursing homes and Notre Dame is one of the best nursing homes in Worcester County. Their staff are great, friendly, reliable, professional and give the up most respect and dignity to your loved one. The staff engage with the residents and make each and every resident feel liked and loved. The nursing staff especially the C.N.A.'s are the backbone to why Notre Dame has the rating it does. The nurses and C.N.A.'s take such good care of each of their residents and love each one of them as their own relative. Thats what makes Notre Dame stand out from the rest. Its not just a job to the staff at Notre Dame it what they were meant to do.Their meal plan has a very good option of food for each meal. All sorts of different ice creams. Notre Dame also has a variety assortment of drinks to choose from unlike many nursing homes which only have the basic few options. There linen and towels and face cloths are soft not rough like many other nursing homes.Their equipment is up to date and not out of date and old and unreliable. If you're looking to place a loved one in a home, look into Notre Dame. Highly recommend! 5 stars.

Dad was a deacon emetrius of a Baptist church in Maine. He especially enjoyed talking about faith stimulating. He also participated in their hospice program. I was so happy to have him t.here

Carol Morello: My mother was there for a short time and I am sorry now I didn't choose somewhere else. The first interaction was the room swap. Because my mother stayed in her chair, they moved her from the first room they assigned to another. A stressful event. The next event, her oxygen. They took her to the dining area and she was feeling ill. Luckily, my husband was there and saw her oxygen tank was empty. Second stressful event. After this, her PA told me to call her anytime for any reason which I thought was nice. One day, I wanted them to check her for a UTI since being a medical professional I know the signs. They said she didn't have one but in the end she did at which point they told me I didn't follow protocol (?). Next, her roommate got a severe upper respiratory infection and ended up in a hospital. I contacted her PA and was scolded by the staff again for not following protocol. Subsequent to this event, the staff failed to inform me or my family about the severity of her roommate' illness and failed to "follow protocol" by gowning up or instituting precautions, exposing us and visitors. Needless to say, my mother got the infection and subsequently passed away. Please review other places before settling here.

Bobby Billionaire: I would like to know why the reviews are so low there has to be something going on that loved ones I've read that report to be abused and threatened people are bed ridden and the CNAs are very abusive and mean I want answers I will be investigating this place I'm a well known lawyer in the state of Massachusetts New York and Rhode island. Unacceptable this facility should be ashamed.
Dawn M: My mother was recently a patient at the Worcester Rehabilitation Center and had a horrible experience. Let me start with my mother who is a fall risk, was given a bed without bed rails. My mother needed to use a bed pan, as she was to weak to walk to the bathroom. When the aide took the bed pan away, urine spilled on the bed. The Aide refused to change the bed. The urine stained sheets stayed on the bed until the following day. My mothers roommate changed the bed for my mother. My mothers last night there she rang the call light as she needed assistance to the bathroom, the aide was extremely rude and pulled on my mothers arm to hurry up, and shoved my mother!!! That is assault. I can't even believe this level of care is out there. We had a team meeting regarding my mothers care. The Nursing director stated she did not have any concerns with my mother being released. I stated I have concerns as my mother has RSV and was not feeling well. The next day my mother was sent to the hospital. We did not receive a phone call from the Rehabilitation Center that our mother was being brought to the Hospital. The Registration Department at the hospital called for information regarding my mother, this is how we were notified.To the aide who worked that evening assisting my mother, you should be ashamed of yourself!!!

Physical abuse of elderly people. They probably are nice to younger people as they can fight back but not so with older ones. File a report with jacho. I let the cops know.

They lied to my family about having the correct size bed for my father and let him sit in a dirty diaper for hours Multiple times at night and would yell at him when they changed it and they're not following giving him meds properly

Administration staff participates in fraudulent billing, refuses to address unsafe conditions for patients, allows neglect and abuse from floor staff. Dietary restrictions not followed. Medications not administered appropriately. Therapeutic modalities not offered, yet billed for. Transportation to medical appointments for patients often canceled effecting continuity of care. Just horrific. Called state DPH, ombudsman, HHS, Medicare, Medicaid, Athena Corporate. Shameful abuse of medically compromised individuals.

Mackenzie Porter: This place is terrible. My dad had a roommate who would blare music and threaten him constantly. To the point where my dad had to sleep out in the hallway in the wheelchair. The roommate received no repercussions. The roommate finally hit my dad and went to the hospital. The staff said the man would not be back on that floor but he was back there after 2 days. The place is so dirty. Barely any running water for a shower and it comes out dirty. There was dried up blood on the walls in the bathroom. The place looks like it hasn’t been cleaned in years. Not a lot of updated equipment or knowledge on equipment that would have been helpful for patients.

John Tsoumas: To whom it may concern. My brother peter tsoumas has been here for two plus years. Hes been at three facility's prior to this one and this one is the best one of all. In the 2.5 years im very satisfied with the care that hes been giving. The staff lead by latanya sims whom is the director of the unit has been most attentive to peters needs. Latanya sims is a huge asset to this organization and hopefully she will remain with peter for years to come. Every time i have a problem i deal with latanya personally and she ALWAYS accommodates peters needs.Sincerely john toumas.

Latonya has made a difference in the quality of life for my family member who currently resides at Worcester Rehab. She is kind and has a wonderful sense of humor which are qualities that have enabled her to help him feel successful and not frustrated. We have been grateful for the hard work she does to help him achieve his goals.

Sheena Skeean: If I could I would give this place negative stars. The staff does not care about the patients. My dad is a current patient there. He has been left in his chair for long periods of time when he cannot sit. It will take them 30 to 45 minutes to change his diaper which is uncalled for and has caused him sores. When calling the facility it takes 10 to 15 minutes or a couple times of calling to get to the floor that you need to be connected to. Mailboxes are always full and can never leave a message. The social worker never gives a call back when you leave a message. They are not good to their patients. If you have an issue it does not get resolved. They have lost my father‘s clothes in The laundry. He has had one shower since being up there and it was an ice cold shower that they gave him. And they do not answer the patients call light in a timely fashion. There has been times where the light will be on for an hour and no one will come in. My father also has mice in his room and nothing is being done about it. The place is an absolute disgrace

Colleen Burr: Make sure you have oversight for any loved one that is sent here. The amount of family intervention, follow-up and advocating that was required for our mother to receive basic health care was exhausting. Maintaining proper insulin levels during post-surgery recovery is critical to reduce infection. It is simply negligent that she did not receive sugar checks and insulin in a timely manner on multiple days. Also inexcusable is the fact that she had to walk with a walker to the nurse’s station at 11 pm to get pain medicine (after waiting 40 mins after pressing call button with no response). It was difficult just to get water regularly.Although the staff were very pleasant, the overall management of care was horrendous. And the communication breakdown here is ridiculous.Mary and Brianna in admissions were helpful, as was nurse Renee. The PT therapist was great (once mom finally received 4 days after admission, as she tested positive for covid before leaving hospital, though no symptoms AND testing negative on day 1 at this place) and the food is excellent.Unfortunately, nice people and good food don’t make up for mismanaged care. I worry for folks who don’t have regular visitors/advocates. So sad.I have the utmost respect for caregivers and have no doubt most employees at this facility do the best they can. Perhaps starting with a basic chart in the patient’s room showing what meds they are on and frequency, as well as a sign off sheet, etc., would be helpful. And not waiting until day 5 to have a meeting to discuss patient care, but rather have this on day 1 or 2; regardless of if that falls on a weekend.

The kitchen management is perhaps the most pathetically mismanaged abomination I ever had the Mispleasure of witnessing. I know there isn’t a god because if there was he would strike lightning on all those responsible. Holy trinity is a case study in how not to run a business. Just typing this paragraph reminds me I need to take my blood pressure meds. Don’t come here even if your life depended on it. Your death would’ve been more dignified than to have supported this travesty. Bye.

I will admit this place is very kind friendly and hospitable I love it here I love the people the nurses the doctors the staff even the kitchen staff!! They give you extras if you ask nicely hint hint!! Facility is very well kept and maintained I would rate it a five-star on cleanliness and neatness and appearance!! When I was staying the walls were being repainted and the ceiling tiles being replaced so they are evolving for a 20 year old facility of healing!! There technology of tvs needs a bit of improvement but are fine for now!! I love the physical therapy people who got me back up and running are PHENOMENAL AND ANGELS IN HUMAN BODIES!! Ellen Whiting: My mother was a patient at Holy Trinity in the rehab for several weeks. She received wonderful care. The staff, particularly the aides were kind and caring. The place is clean, homey and the food is excellent. I was hoping for a placement closer to where I live but Holy Trinity turned out to be perfect! Thank you!
Lori Pepka: As a short term patient, and a young spastic stroke survivor, my impression of HolyTrinity rehabilitation and nursing healthcare facility: I am grateful and highly impressed by the outstanding nursing, nurses’ assistants, and personal care assistants. They are a major part of my recovery as they are proven experts in rehabilitation. The Occupational therapist and physical therapist works well for me in my road of recovery from significant stoke recovery setback by a fall. I truly hope the team at Holy Trinity can get some regional, if not national attention, as a caring skilled rehabilitation center. Sincerely Lori Pepka
Heidi Anderson: I have been a volunteer since last summer. I wanted to spend my free time with elderly in a non paying capacity just visiting and listening to stories of the past. I used to work as a nurses aide in my youth so I empathize with the staff and their nursing responsibilities. I have found the staff to be very courteous and attentive. I have sat with patients in the smaller dining facilities and impressed by the efforts to get residents to eat well and balanced meals. The food looks home made and appetizing. The front lobby has iced water with lemon slices and fresh coffee for visitors and residents all day long so it almost as nice as visiting someone at home. The personal private space is small but not any different than any other facility. This seems to be the hardest adjustment for residents who have lived independently in their own homes. The facilities are immaculate and there is no odor. We all know that isn't always the case in a facility. I've seen a few "oops" moments with residents and the staff is quick and efficient to get the situation cleaned up and privacy protected for the resident. Very discreet staff and I am proud that they honor resident privacy. Very guarded about their residents as if their own family. There is love in this building which is why I keep returning for more. This is a happy place with lovely grounds - a garden and gazebo as well as an on site chapel with eastern orthodox icons. This is where I would like to spend my golden years when the time arrives and recommend it for your family too.
Samantha Cormier: My grandmother just had a short stay at Holy Trinity, everything from the staff to the food was amazing! Very nice and caring atmosphere. She will be back when she has her next knee replaced. Highly recommended.
Kelly Paschopoulos: This place was terrible! My family member was only there for 2 days and both were a nightmare, 2 days was too long,we decided to transfer her. On the day we moving her to a new and very caring rehabilitation, I went in to see her, I sat down in a chair and found an entire bag of prescription meds including narcotics just sitting on a chair ,completely un attended for anyone to take! They didnt know I had them until I told them,and that I took pictures of the large amount of pills anyone could have walked off with ,an entire bag of bubble packed opioids! They pay zero attention to anything there, including your loved ones. This place needs to be investigated at the very least.

Sabine Mende: Holy Trinity Nursing Home is a wonderful place for residents. The staff is friendly and caring. They take good care of the residents by showing much love and affection. The residents are happy to be there as they always have something to look forward to from fun activities to entertaining events. It feels like home!

We told the management about this and told them that we would do her laundry at home. Her clothes still disappeared out of her closet. We found a more reliable nursing home for her to live.

Barbara Ross: When I landed myself at Parsons Hill it will be a time I will never forget. Some bad things alot of good things as well. Choices that I made good or bad are my accomplishments and flaws. Working on my flaws got me where I em today, a better person and friend. I had alot of anger that special CNA'S and NURSES as well as Social Services, Head Nurse Dept to the wonderful Receptionists at the main entrance to the hard working administration office on my side to help. 🔥 If I could give them a 6 star I would. Thank you for all your help.

K&A Larievy: Horrible. This place has kept our loved one for owing money. They have stripped him of all his civil rights and Parsons has treated him worse than an animal. Our loved one suffered a fracture back in their care, his belongings are always stolen and they gave him a slouched bed he falls out of. They have turned away his attorneys and a doctor of his choice. The staff has been over heard cursing at him and we have been told they pushed the door open on him - telling him he has no privacy. Bad bad place.

With a lot of hard work here I feel confident enough to leave Parsons Hill to be on my own again. Michael Dodds: I was a patient at Parsons Hill for a week to rehabilitate my legs. What I loved about Parson Hill was the staff, how professional they are and also how caring they show kindness to all the patients.The rooms are comfortable and the food is good, they provide entertainment such as arts and craft, bingo, moives and they bring in a person to sing music to us. I highly recommend this facility for those in need of health care and rehabilitation. My hats off to you Parsons Hill and keep up the good work.

JC Carreira: My brother was made to stay in this place for some time and I'll tell you the conditions were terrible... You enter the place and have to walk past what they call the "poop Deck" which is appropriately named as the smell is awful. They all hangout outside smoking cigs all day. My brothers roommate continued to get alcohol in the place from visitors. Someone on here said drugs were obtainable too which I can totally see as being possible. I know my brother was there for rehabilitation from losing his leg due to substance abuse but this place is bad, there are other options that seem to take things more seriously and professionally.
Dairenne Gonzalez: As an employee at Parson Hill I love and enjoy coming to work everyday to see my residents and help them in any way possible. It is unfortunate we have a pandemic going on, but we try to come up with new things to do with our residents to make their stay here a better one!
Nana Acheampong: Parsons had my family member there whom they treated very poorly. His leg was swollen due to blood clot and not a single doctor or nurse picked up on it. It took a family member to visit to notice the swell who then contacted emergency services to take him to a hospital. The doctors at this facility do not care about the patients and does not provide services based on the oath they have taken as doctors to care for humanity.

meghan tennant: The residents there do not do anything but sit and be bored as for the TBI rehabilitation it is non existent an there is no physical therapy room no physical therapy exercise equipment and and the executive director hides in his office all day with a big sign that says do not disturb None of the directors want to hear from the residents they act as though it's a burden. They didn't give me my covid pay either! This place only cares about the money coming in. Some of the kitchen staff care and some of the CNA's care but that's it. In the short time I was there I heard nothing but complaints from those residents and when I bring it to the attention of those who need to change it nothing happens The building is so old inside old inside and needs an uplift badly Every department is understaffed. I would never work for that company again.

Marlene Geary: Horrific place - do not let your relatives stay here. My 59 year old uncle entered West Side House after an extended hospital stay and he was able to walk with a walker. They kept him restrained to a chair where he proceeded to lose the ability to walk because he wasn't using his leg muscles (which atrophied). He then lost continence because they wouldn't come to let him go to the bathroom when he had to, so he simply gave up. They also said they were experienced in VA care and they were not. The clothes we bought for him (and had specially monogrammed) never came back from the laundry - only the clothes they bought for him. They fought to keep him when we battled to have him transferred - they said he was assaulting nurses, although we still do not understand how a man with his arms restrained to his chair can assault nurses.Make no mistake - this is a *for profit* institution and their intention is to profit from every patient. Please do not let your family members go to this place.

Rand Alfaris: Dear people (May 21, 2018)After my review yesterday, I received a call from the management requesting a meeting with me to address the points that have been risen in my review. When I posted my review, I had no idea that there would a response, but I was surprised how fast they responded, and how fast they found solutions. I've found the management was very appreciative to what I've said and they were trying their best to attend to the negative issues. They assured me that a certain action has already been taken regarding the hygiene point. Regarding mother's situation, which was my main concern, things were resolved in the way that is better then what I was hoping for. I'm pleased I've chosen this center for mother's rehabilitation period.Dear people (May 20, 2018)I'm in this center, accompanying my mother for 6 days now, therefore my review's based on my subjective observation as a gust not as a patient. Here's what I've noticed:Points of strength:1. Every one is working in this center is very kind and understanding. Except maybe one odd case that I've noticed, with which was dealt properly.2. The meals are made and delivered to one's diet. I've given them mother's food program and they followed it to the letter.3. The physical therapists and the occupational therapists are very good and they will stick to the exact timings if you ask them to follow one.4. A personal from the management is checking on the residents one by one to make sure they are comfortable and have what they need.Points of weakness:1. The phamesy is irresponsible: apparently there is something missing the chain of orders. Every thing is in the charts, but apparently no one reads them before they bring the medication. Mother is under going cancer treatment and she needs to take her medication (chemo and hormone pills) every day at 8 am sharp. Until now no one got perfectly right, sometimes too late and sometimes missing a pill. And every day I have to remind them. Today morning I remind them again, the nurse told me, "see, I don't even know that!" I said, it's in the chart! The chart itself is still manually written! It amazed me in the 21 century, they still do manual charts! Also there is no schedule for the pain killers. Or to be more specific, there is a schedule but I have to remind them every time. Sometimes mother is in pain and they are not sure what they should give her and at what dose they should give her. Until I asked them please tell me what the system of giving those pain killers is.2. The center is way under staff on weekends-especially at night: I don't understand, are the bowel movements of human been take time off at weekends? Are the needs of these seniors and patients any less on weekends?Why is the center is under staff on weekends. Two assistants only for the entire floor. And I've noticed that it takes even a longer time for an assistant to come for the patients and seniors who are buzzing for help.3. Can't find hot water in the night! When I ask for it, they microwave cup of water. Microwaving water is very dangerous for a cancer patient.4. The hygiene should be at maximum level. Which unfortunately it isn't. When they empty the bed pan for some patients, they washed it in the same hand/face washer sink without scrubbing it after. There should be a proper place for washing the bed pans or they should scrub the sink after each time they do that. I couldn't use the sink or let mother use it after they did so with mom's roommate. I complained to them to clean it.5. There are a lot of noise at night and minimum privacy and very much inconvenience with lights arrangements because the rooms are shared. If the roommate can't sleep at the sleeping hours or wants to hear loud tv because of the hearing impairment while the other is asleep, what would happen here! You can imaging the situation, right? What would it take if they divided the rooms not with just a curtain but with wooden wall? Mother couldn't sleep a whole night straight because of the noise and nurses coming and going for her roommate.
Lee Leonard: Great care, my mother in law spent a few years there and was recently under Hospice care which was top notch. She passed today God live her.
